College student Fiona Wagner (Katheryn Winnick) fears for her safety after hearing of a local murder where a young girl was forced to choose which one of her parents should die.

When Fiona receives a message from the killer on the anniversary of her mother's suicide, she comes to believe her mother was faced with a similar decision: to kill herself or harm another. When Fiona tries to speak to one of the victims, all he will say is that the killer knew his name and spoke of the ramifications of his choices. Words that match the note Fiona's mother left behind.

Fiona returns to the hotel where her mother died. Closing a door, she sees a man's reflection in a mirror. The man (Lenny Von Dohlen) tells her he could have her arrested for trespassing, but Fiona defends herself and escapes.

Clues led Fiona to a former juvenile detention center where experiments of choice therapy were conducted on the youth. Fiona and her father, the local sheriff, soon learn of a connection between the victims. When the killer asks Fiona to meet him at the abandoned juvenile detention building, she is drugged and awakens to a nightmare she could not have foreseen.
